- Review and validate medication orders prescribed for admitted patients.
- Check dosage appropriateness, drug interactions, allergies, contraindications, and duplication of therapy.
- Monitor patient medication therapy and recommend changes when necessary.
- Prepare and monitor high-risk medications, IV admixtures, and emergency drugs.
- Monitor adverse drug reactions (ADR) and medication errors; report findings.
