A hospital clinical pharmacist is a medication therapy expert who works directly with doctors and nurses to optimize patient drug regimens, improve safety, and enhance clinical outcomes. Their primary duties include reviewing medication orders, providing patient counseling, adjusting dosages, participating in rounds, and ensuring antimicrobial stewardship.
Key Responsibilities

- Clinical Review & Monitoring: Evaluate patient medication profiles for efficacy, safety, potential interactions, and contraindications.
- Direct Patient Care: Conduct bedside counseling, obtain medication histories, and educate patients/families on treatment regimens.
- Cooperative Practice: Participate in daily medical ward rounds to recommend, adjust, and monitor drug therapy (e.g., in ICU, oncology).
- Drug Safety & Policy: Prevent medication errors, manage adverse drug reactions, and implement hospital-wide antimicrobial stewardship protocols.
- Dosage Adjustment: Calculate and customize drug dosages,



especially for specialized treatments or high-risk patients.

Required Qualifications & Skills

- Education: Doctor of Pharmacy (Pharm.D.) degree.
- Licensure: Current licensure/registration to practice pharmacy in the state.
- Experience: Often requires post-graduate residency training (PGY1) or 2+ years of hospital experience.
- Skills: Strong critical thinking, communication, collaboration, and specialized pharmacological knowledge.

Common Working Conditions

- Collaborates with a multidisciplinary team (physicians, nurses, specialists).
- May involve shift work, weekends, or on-call arrangements.
