The technical linchpin. Owns architecture, the backend, the cloud foundation, the tenancy model, and security.

You will

· Own the system architecture and the database-per-clinic data layer, control plane, and migration tooling.

· Build the RIS backend in TypeScript/NestJS and the AWS foundation in Terraform.

· Integrate the PACS (Orthanc) — Modality Worklist, DICOMweb, S3 storage — and the HL7 engine (Mirth).

· Set the security baseline: encryption, secrets, RBAC, audit logging.

Must have

· 6+ years building production web backends; robust TypeScript/Node (or readiness to lead in it).

· Deep PostgreSQL, including multi-tenant or multi-database patterns and connection pooling.

· Hands-on AWS (ECS/Fargate, RDS, S3, IAM), Docker, Terraform, CI/CD.

· A security-first instinct and clean REST API design.

Strongly preferred

Healthcare data experience; exposure to DICOM, HL7, or PACS/RIS systems.

Built and run a multi-tenant SaaS in production.
