Assess the physical, mental, and functional health status of adult and elderly patients at home.
Develop and implement individualized care plans based on patient needs and medical history.
Monitor and manage chronic diseases such as diabetes, hypertension, heart disease, and COPD.
Prescribe and administer medications as authorized and monitor for side effects or adverse reactions.
Perform comprehensive health assessments and order or interpret diagnostic tests when appropriate.
Provide wound care, catheter care, and other advanced nursing procedures in the home setting.
Educate patients and family members about disease management, medications, nutrition, and healthy lifestyle practices.
Collaborate with physicians, therapists,



and other healthcare specialists to ensure coordinated care.
Recognize changes in the patient's condition and initiate timely interventions or referrals when necessary.
Maintain accurate clinical documentation and ensure high-quality, secure, and patient-centered home healthcare services.
