- Type and prepare discharge summaries, medical reports, operation notes, consultation notes, and investigation reports accurately.
- Transcribe consultants' dictated notes and ensure correct medical terminology and formatting.
- Maintain confidentiality of patient records and comply with hospital privacy policies.
- Verify patient information and ensure all medical documents are complete and error-free before submission.
- Coordinate with consultants, nursing staff, MRD, and other departments for timely completion of documentation.
- Upload and maintain medical reports in the Hospital Information System (HIS) and Electronic Medical Records (EMR).
- Ensure timely completion of discharge summaries to facilitate smooth patient discharge.
- Maintain records of typed documents and ensure proper filing and retrieval of medical records.
- Support NABH documentation requirements and participate in quality audits related to medical documentation.
- Perform any other duties and responsibilities assigned by the Management or Reporting Manager from time to time.

Pay: ₹20,000.00 - ₹25,000.00 per month
