Roles & Responsibilities An ED Facility Medical Coder is responsible for reviewing Emergency Department (ED) documentation and accurately assigning medical codes for facility billing, reimbursement, compliance, and reporting purposes.

Key Responsibilities:

1. Medical Coding & Charge Capture

- Review Emergency Department medical records, physician notes, nursing documentation, diagnostic reports, and treatment records.
- Assign accurate ICD-10-CM, CPT/HCPCS, and facility-specific codes based on documented services.
- Determine appropriate ED Evaluation & Management (E/M) facility levels according to organizational guidelines.
- Ensure accurate coding of diagnoses, procedures, treatments, medications, and ancillary services.

1. Documentation Review

- Analyze clinical documentation for completeness, specificity, and medical necessity.
- Identify missing, incomplete, or conflicting documentation.
- Query providers and clinical staff when clarification is required for accurate code assignment.

1. Compliance & Quality Assurance

- Maintain compliance with CMS, Medicare, Medicaid, HIPAA, and payer-specific regulations.
- Follow AHIMA, AAPC, and official coding guidelines.
- Ensure coding accuracy to minimize compliance risks and audit findings.
- Participate in internal and external coding audits.

1. Revenue Cycle Support

- Support accurate and timely claim submission.
- Identify coding-related denials and assist in denial resolution.
- Work closely with billing, revenue cycle, CDI, and clinical teams to improve reimbursement accuracy.




- Monitor coding edits and ensure appropriate claim corrections.

1. Productivity & Performance

- Meet established productivity and quality benchmarks.
- Maintain turnaround times for coding Emergency Department encounters.
- Track and report coding trends, errors, and opportunities for improvement.

1. Medical Record Analysis

- Review:

- ED physician documentation

- Nursing assessments

- Triage notes

- Laboratory results

- Radiology reports

- Medication administration records

- Procedures performed in the

1. Continuous Learning

- Stay current with annual ICD-10-CM, CPT, and HCPCS updates.
- Attend coding education sessions and training programs.
- Keep certifications active and maintain continuing education requirements.

Required Skills

- Robust knowledge of Emergency Department coding guidelines.
- Expertise in ICD-10-CM, CPT, HCPCS, and E/M coding.
- Understanding of medical terminology, anatomy, physiology, and pharmacology.
- Familiarity with EMR/EHR systems and encoder software.
- Strong analytical, communication, and attention-to-detail skills.
- Ability to work independently while maintaining high coding accuracy.

Preferred Qualifications

- CPC, CCS, CCA, CIC, or equivalent coding certification.
- Experience in Emergency Department/Facility Coding.
- Knowledge of UB-04 billing and facility reimbursement methodologies.
- Familiarity with coding audits, denial management, and compliance standards.
