Role Description The Clinical Pharmacist role at The Lifeline Hospitals is a full-time, on-site position based in Chennai.

The Clinical

Pharmacist will review and verify medication orders, ensure safe and effective use of medicines, and provide pharmaceutical care in collaboration with physicians, nurses, and other healthcare professionals. Daily responsibilities include assessing medication therapy for appropriateness, monitoring for drug interactions and adverse effects, and counseling patients and families on proper medication use and adherence. The role also involves overseeing compounding processes, maintaining accurate medication records,



and adhering to hospital policies and regulatory standards.

The Clinical

Pharmacist will contribute to clinical rounds, participate in quality improvement initiatives, and support medication safety education for staff.

Qualifications

Demonstrated proficiency in Pharmacy Practice and comprehensive knowledge of Pharmacy operations.

Strong foundation in Pharmaceutical Care and Medicine to support evidence-based clinical decision-making.

Hands-on experience with Compounding and safe preparation of medications in accordance with standards.

Relevant degree in Pharmacy (B.Pharm or M.Pharm) from an accredited institution; valid pharmacist registration/licensure as required.

Experience in a hospital or clinical setting, preferably in clinical pharmacy or patient-focused medication management.

Ability to interpret prescriptions, clinical protocols, and laboratory data, and communicate recommendations clearly to the healthcare team.

Attention to detail, strong ethical judgment, and commitment to patient safety and confidentiality.

Willingness to work on-site in Chennai, collaborate in multidisciplinary teams, and adapt to a fast-paced clinical environment.
