- Use clinical pharmacy prescription approval module to review and authorize medication orders digitally.
- Detect and mitigate medication errors proactively.
- Perform IP and OP prescription audits, initiate corrective and preventive actions (CAPA).
- Develop, prepare, and revise the hospital drug formulary.
- Audit on secure usage of narcotics and psychotropic drugs.
- Participate in daily ward rounds with clinical teams.
- Ensure adherence to hospital medication management policies.
- Coordinate for Antimicrobial Stewardship Program (AMSP).
- Serve as member of Hospital Infection Control,Pharmacy and Therapeutics Committee, Ethics, and Quality Assurance committees.
- Attend meetings and present data on medication management indicators.
- Conduct pharmacovigilance including Adverse Drug Reaction (ADR)
monitoring and Reporting with CAPA.
- Provide staff training on medication management. (MOM).
- Evaluate medication orders for appropriateness of dose, route, frequency, and duration throughout hospital stay.
- Ensure rational prescription of drugs to maximize efficacy and minimize adverse effects.
- Coordinate with doctors to adjust medication doses for renal and hepatic impairment.
- Deliver drug information services, answering queries from healthcare professionals.
- Monitor surgical antibiotic prophylaxis and ensure compliance with protocols.
- Drug Reconstitution as per requirements.
