The Cancer Patient Care Navigator/Coordinator will play a critical role in supporting cancer patients throughout their treatment journey. This position is responsible for guiding patients through the complexities of the healthcare system, ensuring they receive timely care, and addressing any barriers to treatment.

The Care

Navigator/Coordinator acts as a connection between patients, families, and healthcare providers to facilitate communication and coordination of care.

Key Responsibilities

Patient

Support and Education

- Provide personalized support to cancer patients and their families.
- Educate patients about their diagnosis,



treatment options, and available resources.
- Assist patients in understanding their treatment plans and managing expectations.
- Care Coordination:
- Coordinate appointments, tests, treatments, and follow-up care with various healthcare providers.
- Ensure seamless communication between the oncology team, primary care physicians, and other specialists.
- Track and monitor patient progress and treatment milestones.
- Documentation and Reporting:
- Maintain accurate and up-to-date patient records.
- Document all patient interactions, interventions, and outcomes.
- Generate reports on patient progress and program effectiveness.
- Collaboration and Communication:
- Work closely with the oncology team to develop and implement individualized care plans.
- Facilitate effective communication between patients, families, and healthcare providers.

Qualifications

- Bachelor’s degree in Psychology, Nursing, Pharm D.
- Minimum of 1-2 years of experience in oncology, patient navigation, or a related healthcare role.
- Solid knowledge of cancer diagnosis, treatment, and care processes.
- Excellent communication, organizational, and interpersonal skills.
- Compassionate, patient-centered approach to care.
