1. Patient Medication Review

- Review patient prescriptions for accuracy and appropriateness.
- Identify drug interactions, contraindications, and duplicate therapies.
- Recommend dosage adjustments based on patient condition.

2. Clinical Pharmacy Services

- Participate in ward rounds with physicians.
- Provide evidence-based medication recommendations.
- Monitor the effectiveness and safety of drug therapy.
- Assist in optimizing treatment plans.

3. Medication Counseling

- Educate patients on:
- Proper medication usage
- Dosage and timing
- Possible side effects
- Drug-food and drug-drug interactions
- Storage and disposal of medicines

4. Adverse Drug Reaction (ADR) Monitoring

- Detect and document adverse drug reactions.
- Report ADRs to pharmacovigilance programs.
- Recommend preventive measures.

5. Medication Safety

- Promote rational drug use.
- Reduce medication errors.
- Ensure compliance with hospital medication policies.

6. Drug Information Services

- Provide accurate drug information to:
- Doctors
- Nurses
- Patients
- Other healthcare professionals

- Answer queries regarding drug indications, dosages, interactions, and safety.

7. Therapeutic Drug Monitoring (TDM)

- Monitor drugs with a narrow therapeutic index.
- Interpret laboratory values.
- Recommend dose modifications when necessary.

8. Medication Reconciliation

- Compare patients' previous medications with current prescriptions.
- Prevent omissions, duplications, and dosing errors during admission, transfer, and discharge.

9. Clinical Documentation

- Maintain patient medication records.
- Document pharmaceutical interventions.
- Prepare discharge medication summaries.

10. Participation in Clinical Research

- Assist in clinical trials.




- Collect and analyze research data.
- Ensure compliance with Valuable Clinical Practice (GCP).

11. Pharmacovigilance

- Monitor drug safety.
- Report medication-related problems.
- Contribute to national pharmacovigilance programs.

12. Quality Improvement

- Participate in medication audits.
- Conduct Drug Utilization Evaluation (DUE).
- Support NABH/JCI quality initiatives.
- Develop Standard Operating Procedures (SOPs).

13. Antibiotic Stewardship

- Monitor antibiotic prescribing practices.
- Promote appropriate antimicrobial use.
- Help reduce antimicrobial resistance.

14. Inventory and Formulary Management

- Assist in formulary development.
- Evaluate new medicines.
- Monitor high-risk and high-cost medications.
- Reduce medication wastage.

15. Healthcare Team Collaboration

- Work closely with:
- Physicians
- Nurses
- Dietitians
- Physiotherapists
- Laboratory staff

- Participate in multidisciplinary patient care discussions.
