Job Title Health Coach Location Hyderabad Banjara Hills About Us HEAPS is one of the fastest-growing health tech start-ups in India today With over 5 million patient interactions to date HEAPS is revolutionizing how insurers doctors and patients experience healthcare Our AI-driven end-to-end care management platform coordinates tracks and monitors patients health empowering them with the right information to make timely decisions about their care Backed by marquee investors with over 7 4 million raised in Series A funding HEAPS is on a global scaling path and is already present in 4 countries HEAPS is looking for empathetic Care Coaches who are passionate about monitoring patients with chronic conditions using HEAPS AI-driven care management platform Role Overview As a Care Coach for Chronic Care Management you will play a crucial role in improving the well-being of patients with chronic conditions Your primary responsibilities will include monitoring and tracking patient health progress while offering guidance support and encouragement to help them adhere to their personalized care plans By ensuring compliance with treatments medications and lifestyle modifications you will contribute to better disease management and improved patient outcomes Responsibilities Engage with patients over various touchpoints to understand their current medical status past medical surgical history medications lab results etc Identify additional patient needs medications labs dietitian services physiotherapy etc Assess patients social determinants and lifestyle status Ensure patients provide complete medical histories and monitor adherence to care plans Offer psychosocial support to help patients cope with chronic acute or terminal illnesses Monitor patient progress and evaluate health improvements Address patient queries using pre-defined FAQs Educate patients about their chronic conditions treatment options and the importance of care plan adherence Coach patients on the importance of compliance with labs medications and diet Ensure accurate and efficient patient data management Eligibility Criteria Front Desk Expertise in Hospitals Proven experience handling patient interactions appointments and overall coordination at a hospital front desk OR Strong Communication in Telecom A background in telecom services with excellent communication skills OR Call Center Experience in Healthcare Hands-on experience managing patient calls within a hospital setting ensuring prompt and efficient service Skills Strong knowledge of medical conditions and medical terminologies Effective patient medical history-taking skills Coaching patients to comply with medical parameters Clear and articulate communication with patients Active listening skills and compassion Passionate about patient care Ability to engage patients in conversations on various topics Grievance resolution and conflict-handling abilities Competency in managing recording and storing patient information Analytical mindset to interpret health data and trends effectively Positive attitude with a naturally pleasant disposition Team player who can collaborate across departments Qualifications Minimum bachelor s degree Pharm D B Sc M Sc BA with Hospital experience Proficiency in English Hindi and Telugu Job Type Full-time Pay 20 000 00 - 25 000 00 per month Benefits Health insurance PF Schedule Monday to Saturday Language English Telugu are mandatory Other regional languages are beneficial Location Ability to reliably commute or plan to relocate before starting work is required Application Question s Immediate Joining Job Type Full time Pay 20 000 00 - 25 000 00 per month Benefits Health insurance Provident Fund Experience total work 1 year Preferred Work Location In person
