ICU Staff Nurse

Department: Intensive Care Unit (ICU)
Reports To: ICU In-Charge / Nursing Superintendent
Location: Hopewell Hospital, Ranchi

Position Summary

The ICU Staff Nurse is responsible for providing safe, evidence-based, compassionate, and continuous nursing care to critically ill patients requiring intensive monitoring and advanced life support. The role demands excellent clinical judgment, rapid response capability, meticulous documentation, and close collaboration with intensivists, consultants, respiratory therapists, and the multidisciplinary healthcare team.

The ICU Nurse shall ensure patient safety, infection prevention, accurate medication administration, timely escalation of deterioration, and adherence to NABH standards and hospital protocols.

Key Responsibilities

1. Critical Patient Care

- Provide comprehensive nursing care to critically ill patients.
- Perform complete patient assessment at admission and during every shift.
- Monitor vital signs continuously.
- Recognize early signs of deterioration.
- Initiate emergency interventions within nursing scope.
- Escalate immediately to the intensivist or duty doctor when required.
- Maintain airway, breathing and circulation.

2. ICU Monitoring

Monitor and document:

- Heart Rate
- Blood Pressure
- Respiratory Rate
- Oxygen Saturation
- ECG Rhythm
- Temperature
- Urine Output
- CVP (where applicable)
- ICP monitoring (if applicable)
- Ventilator Parameters
- Blood Sugar
- Neurological Status (GCS)
- Pain Assessment
- Sedation Scores

3. Ventilator Care

- Assist in intubation.
- Prepare ventilators before admission.
- Monitor ventilator settings.
- Ensure humidification.
- Perform ET tube care.
- Maintain ventilator-associated pneumonia (VAP) bundle.
- Assist during extubation.
- Monitor post-extubation patients.

4. Medication Management

Safely administer:

- IV medications
- Vasopressors
- Sedatives
- Inotropes
- Analgesics
- Insulin Infusions
- Blood Products
- Antibiotics
- Electrolytes
- Emergency Drugs

Ensure:

- Five Rights of Medication
- Double verification for high-alert drugs
- Correct dilution
- Infusion pump programming
- Medication documentation

5. Emergency Response

Actively participate in:

- Code Blue
- CPR
- ACLS Protocols
- Rapid Response Team activations
- Massive Transfusion Protocol
- Cardiac Arrest Management
- Shock Management
- Sepsis Bundle

6. Equipment Management

Operate safely:

- Ventilators
- Syringe Pumps
- Infusion Pumps
- Defibrillator
- Multi-parameter Monitor
- ECG Machine
- Suction Apparatus
- ABG Machine
- Crash Cart
- Oxygen Delivery Systems

Ensure:





- Equipment functionality
- Daily checks
- Immediate reporting of malfunction

7. Infection Control

Strict adherence to:

- Hand Hygiene
- PPE Protocols
- Isolation Guidelines
- Central Line Care Bundle
- Catheter Care Bundle
- Ventilator Bundle
- Biomedical Waste Segregation
- Sterile Procedures
- Needle Stick Injury Protocol

8. Documentation

Maintain accurate documentation of:

- Nursing Notes
- Intake Output Chart
- Medication Chart
- ICU Observation Chart
- Ventilator Chart
- Pressure Injury Assessment
- Fall Risk Assessment
- Consent Verification
- Shift Handover
- Incident Reports

Complete documentation in HMIS immediately after patient care activities.

9. Patient & Family Communication

- Educate family regarding ICU protocols.
- Explain nursing procedures.
- Provide compassionate communication.
- Coordinate family visitation.
- Escalate concerns appropriately.
- Maintain confidentiality.

10. Admission & Transfer

Coordinate:

- ICU Admission
- Bed Preparation
- Patient Transfer
- OT to ICU Handover
- ICU to Ward Transfer
- Inter-hospital Transfer

Ensure complete clinical handover using SBAR.

11. Clinical Procedures

Assist in:

- Central Line Insertion
- Arterial Line
- PICC Line
- Chest Tube Care
- Tracheostomy Care
- Dialysis
- Bronchoscopy
- Bedside Procedures
- Lumbar Puncture
- Temporary Pacemaker

12. Nursing Care

Provide:

- Pressure Area Care
- Oral Care
- Eye Care
- Catheter Care
- Feeding Tube Care
- Enteral Feeding
- Passive Exercises
- Position Changes
- Skin Assessment
- Fall Prevention

13. Shift Responsibilities

- Receive complete handover.
- Check crash cart.
- Verify emergency drugs.
- Verify oxygen supply.
- Check equipment readiness.
- Review pending investigations.
- Ensure medication availability.
- Prepare ICU before next shift.

14. Coordination

Coordinate with:

- Intensivists
- Consultants
- Respiratory Therapist
- Physiotherapist
- Laboratory
- Blood Bank
- Radiology
- Pharmacy
- Biomedical Engineering
- Housekeeping
- CSSD

Required Qualifications

- GNM / B.Sc. Nursing (Mandatory)
- Valid State Nursing Council Registration
- Basic Life Support (BLS)
- ACLS Certification (Preferred)
- ICU Certification (Preferred)

Experience

- Junior ICU Nurse: 0–2 years (ICU exposure preferred)
- ICU Staff Nurse: 2–5 years ICU experience
- Experience in handling ventilated and critically ill patients is preferred.

Pay: ₹15,000.00 - ₹35,000.00 per month
