Key Responsibilities:
Assess and monitor physical, emotional, and spiritual needs of patients receiving palliative care.
Develop and implement individualized care plans in collaboration with patients, families, and the healthcare team.
Administer medications and treatments for symptom control and pain relief.
Educate patients and families on disease progression, care options, and end-of-life planning.
Provide psychosocial support and counseling as needed.
Coordinate care with physicians, social workers, chaplains, and other healthcare specialists.
Advocate for the patient's wishes and ensure dignity and comfort are maintained.
Maintain accurate and timely documentation of care provided.
Participate in family meetings, care planning conferences, and case reviews.
Support bereavement care for families after patient death, when applicable.
