Job Summary

We are looking for qualified and compassionate B.Sc. Nurses / DGNM to provide

skilled nursing care to patients in their homes. The Home Care Nurse will

be responsible for delivering clinical care, monitoring patient health,

administering treatments, educating family caregivers, and ensuring

compliance with healthcare standards to improve patient outcomes and

quality of life.

Key Responsibilities

Clinical Care

 Assess patient health status and develop nursing care plans in

coordination with Homecare DMO / Physicians.

 Monitor vital signs, symptoms, and overall patient condition.

 Administer medications, injections, IV infusions, and prescribed

treatments.

 Provide wound care, catheter care, tracheostomy care, colostomy care,

PEG feeding, and other specialized nursing procedures.

 Manage patients requiring oxygen therapy, ventilator support,

BiPAP/CPAP, and critical care services at home.

 Maintain infection control and patient safety standards.

Patient Monitoring & Documentation

 Record patient assessments, treatments, medications, and progress

notes accurately.

 Escalate clinical concerns and emergencies promptly to the treating





physician and supervisor.

 Prepare daily nursing reports and maintain patient records as per

Hospital protocols.

Patient & Family Education

 Educate patients and caregivers regarding disease management,

medication compliance, nutrition, mobility, and hygiene.

 Train family members in basic caregiving techniques when required.

 Provide emotional support and counseling to patients and families.

Care Coordination

 Coordinate with doctors, physiotherapists, dietitians, and other

healthcare professionals.

 Participate in care reviews and multidisciplinary discussions.

 Ensure smooth transition of care after hospital discharge.

Emergency Management

 Identify and respond appropriately to medical emergencies.

 Perform Basic Life Support (BLS) when required.

 Arrange emergency medical assistance when necessary.

 Coordinate with Homecare & Emergency Dept.

Pay: From ₹40,000.00 per month
