Key Roles & Responsibilities

- Review prescriptions, perform medication reconciliation, and ensure the safe and rational use of medicines.
- Identify medication-related problems, adverse drug reactions (ADRs), and recommend appropriate interventions to the healthcare team.
- Participate in clinical rounds, antimicrobial stewardship, formulary management, and medication safety initiatives.
- Educate patients and healthcare professionals on medication use, drug interactions, and pharmacotherapy.
- Maintain accurate documentation and ensure compliance with NABH standards, hospital policies, and clinical pharmacy protocols.

Preferred Candidate Profile

- Qualification: Pharm.D
- Experience: Freshers to 1 year of experience can apply.
- Good knowledge of pharmacotherapy, medication safety,



and clinical pharmacy practices.
- Basic knowledge of Hospital Information Systems (HIS), NABH standards, and clinical documentation.
- Robust communication, analytical, and interpersonal skills.
- Willingness to work in rotational shifts.
