- Manage the day-to-day operations of the Medical Records Department.
- Ensure accurate collection, assembly, coding, indexing, filing, scanning, retrieval, and archival of patient records.
- Verify completeness of medical records, discharge summaries, consent forms, operative notes, nursing records, and investigation reports.
- Coordinate with consultants and clinical departments for completion of deficient medical records.
- Ensure confidentiality and security of patient information as per hospital policy and legal requirements.
- Supervise medical record coding using ICD standards and maintain disease/procedure indexes.
- Maintain physical and electronic medical records (HIS/EMR).
- Handle medico-legal records and provide records only to authorized personnel.
- Coordinate with TPA, insurance, legal authorities, and patients for record requests as per policy.
- Prepare daily,



monthly, and annual MRD statistics and management reports.
- Ensure compliance with NABH documentation standards and support internal/external audits.
- Develop and implement MRD SOPs, policies, and record retention schedules.
- Train and evaluate MRD staff on documentation quality and confidentiality practices.
- Monitor departmental KPIs and initiate quality improvement activities.
- Coordinate with Quality, Nursing, Billing, and Clinical Departments for documentation compliance.

Pay: ₹18,000.00 - ₹25,000.00 per month
