Position: Clinical Pharmacist

FUNCTIONAL RESPONSIBILITIES

· To check for the appropriateness of Prescription including Correct drug, Correct dose, formulation, route, strength and frequency

· To inspect for Therapeutic duplication.

· To check and verify the indent and audit for Medication errors

· To examine and report drug interactions among patients

· to examine and report real or potential drug allergies

· To examine food versus drug interaction among the patients.

· To check the home medication and registering them in a prescribed format with the approved consent from DTC Chairperson.

· To respond all Drugs related query from Health Care Professionals.

· It is must to monitor ADR (Adverse Drug Reaction) and documenting the same.

· To conduct In-Patient ward rounds for checking the Drug charts and discharge summary

· To verify narcotic indents on daily basis

· To participate in drug formulary updates in accordance with its addition and deletion.

· To Prepare the Chemo Admixture whenever required according to the defined protocols.

· To Prepare and reporting High-end Antibiotic usage on daily basis.

· To report antibiotic consumption on a monthly basis.

· Shall have periodical rotational duty at OP Pharmacy and responsible for prescription auditing and patient counseling.

· It must to monitor guidelines in Storage and handling of narcotic drugs





· To monitor the cut strip medication protocols as per SOP guidelines wherever required.

· It is mandatory to check monthly medications stock and short expiry at wards, emergency crash carts, external disaster kit and ambulance kit. Also to oversee the arrangements and labelling of drugs as per protocol.

- To identify and remove the recalled drugs as and when received an internal communication within a stipulated time.

· It is essential to make rounds along with Consultant or Doctor upon their preference.

- Shall follow the SOP and Service guidelines as per the said protocols prevailing in practice.
- To participate in all departmental training and quality initiative measures that is in practice across locations.
- To maintain ethical practices and diplomacy at all levels that we are dealing in our day to day transactions.

· To follow all the rules and regulations of the organization prevails in practice from time to time.

GENERIC RESPONSIBILITIES

- Job Knowledge: Proficient enough in both the Technical & Functional levels for the given role. Conceptual skills and understanding of Business model are utmost important. Shall know the practices in Industry widely followed from time to time.
- Quality Consciousness: Specific action is always aimed at improving quality of service, its processes and other activities associated with it.
- Cost Consciousness: Implementing Cost Control Measures and its monitoring.

Pay: Up to ₹21,000.00 per month
