Job Description:

- Should be able to study patient’s Medical condition and design care plans
- Conduct care team meetings
- Internal monitoring of the data and suggesting tactics to improve care and achieve practice goals.
- Should be able to analyze patient data recorded in software and define the care plans for high-risk chronic disease patients.
- Define Risk level of patient population based on their Medical condition.
- Should be able to Communicate with Doctor’s Office on phone call or via email to coordinate the patient care.
- Attend the virtual learning session and enhance the project knowledge and disseminate it within the Team

Requirement / Qualification:

- Education Background - Graduate Nurses, Medical Officer or Other Medical Background
- Proficient in using Computer and Software Tools
- Students Preparing for USMLE are preferable but not mandatory
- Positive Verbal and written communication skills
- Quick Learner

Shift Timings:

- Day Shift- 8:30 AM to 5:30 PM
- Day-Noon- 12:00 PM to 9:00 PM
- Day-Noon- 1:00 PM to 10:00 PM
- Day-Noon- 2:00 PM to 11:00 PM
