Counselling Psychologist / Case Manager (Minimum 2 Years of Experience)

Roles and Responsibilities

- Conduct psychological assessments and mental status examinations under the supervision of psychiatrists.
- Perform psychosocial assessments for new admissions and outpatients.
- Develop individualized care plans in collaboration with psychiatrists and the multidisciplinary team.
- Provide individual, family, and group counselling sessions.
- Deliver evidence-based psychological interventions for patients with depression, anxiety, stress-related disorders, substance use disorders, psychosis, personality disorders, and other mental health conditions.
- Conduct crisis intervention and provide emotional support during psychiatric emergencies.
- Monitor patients' psychological progress and update treatment plans accordingly.
- Maintain accurate clinical documentation, progress notes, and case records.

Case Management Responsibilities

- Coordinate patient care from admission to discharge.
- Act as the primary point of contact between patients, families, psychiatrists, psychologists, nurses, social workers, and other healthcare professionals.
- Prepare and implement individualized discharge and follow-up plans.
- Ensure continuity of care through regular follow-up appointments and telephonic follow-ups.
- Coordinate referrals to rehabilitation centres, vocational training programmes, de-addiction services, and community mental health resources when required.
- Monitor treatment adherence and encourage medication compliance.
- Identify patients at risk of relapse and initiate timely interventions.

Family Support and Psychoeducation

- Educate patients and caregivers about mental illnesses, treatment plans, medications, relapse prevention, and recovery.




- Conduct family counselling sessions to improve communication and caregiver support.
- Help families develop healthy coping strategies and reduce caregiver burden.

Multidisciplinary Team Coordination

- Participate in daily ward rounds and clinical case discussions.
- Present patient progress during review meetings.
- Coordinate with psychiatrists, nurses, occupational therapists, social workers, and other departments to ensure comprehensive patient care.
- Escalate high-risk cases to the treating psychiatrist immediately.

Patient Safety and Risk Management

- Assess suicide risk, self-harm risk, aggression, and vulnerability.
- Monitor high-risk patients and report concerns promptly.
- Ensure patient confidentiality and maintain professional ethics.
- Follow the Mental Healthcare Act, hospital policies, and clinical protocols.

Rehabilitation and Recovery

- Assist patients in developing coping skills, social skills, and independent living skills.
- Facilitate occupational, recreational, and vocational rehabilitation programmes.
- Encourage participation in therapeutic activities and recovery-oriented interventions.
- Support patients during community reintegration and return-to-work planning.

Documentation and Quality

- Maintain complete and timely clinical records.
- Prepare admission assessments, counselling notes, discharge summaries, and follow-up reports.




- Ensure compliance with hospital quality standards, accreditation requirements, and documentation protocols.
- Participate in clinical audits and quality improvement initiatives.

Awareness and Community Outreach

- Conduct mental health awareness programmes in schools, colleges, industries, and corporate organizations.
- Participate in community mental health camps and screening programmes.
- Promote mental health education and reduce stigma through awareness activities.

Administrative Responsibilities

- Coordinate appointments and patient reviews.
- Maintain counselling schedules and follow-up trackers.
- Assist in preparing departmental reports and patient statistics.
- Support NABH, MHRB, and other regulatory compliance activities as required.

Required Skills

- Excellent counselling and communication skills.
- Strong case management and coordination abilities.
- Knowledge of psychiatric disorders and psychological therapies.
- Crisis intervention and suicide risk assessment skills.
- Ability to work effectively in a multidisciplinary team.
- Good documentation and report-writing skills.
- Empathy, professionalism, and ethical practice.
- Proficiency in MS Office and Hospital Information Systems (HIS).

Qualification

- Master's Degree in Psychology (Clinical/Counselling/Applied Psychology).
- Minimum 2 years of experience in a psychiatric hospital, mental health centre, or de-addiction facility.
- Registration with the appropriate professional body (if applicable).
- Knowledge of the Mental Healthcare Act, 2017 and patient rights is preferred.

Pay: ₹18,000.00 - ₹25,000.00 per month
