The Junior Medical Coder is responsible for reviewing and accurately assigning diagnostic, procedural, and service codes for patient records using standard classification systems such as ICD-10-CM, CPT, and HCPCS. This role ensures compliance with coding guidelines under the supervision of senior coding staff. The Junior Medical Coder works to develop coding proficiency and supports the revenue cycle processes.
Key Responsibilities:
- Code Assignment:
- Assign and sequence ICD-10, CPT, and HCPCS codes for diagnoses, procedures, and services with guidance from senior coders.
- Ensure coding follows official coding guidelines and payer regulations with oversight.
- Request assistance from senior coders when resolving coding discrepancies through review of patient records and physician notes.
- Chart Audits and Quality Assurance:
- Assist in quality assurance reviews of coded records to ensure accuracy and compliance.
- Identify coding questions or potential errors and escalate to senior coding staff.
- Support proper documentation review of patient encounters.
- Revenue Cycle Support:
- Assist the billing department with basic coding questions related to claim issues.
- Support the revenue cycle team under senior coder direction.
- Compliance and Education:
- Comply with HIPAA, CMS (Centers for Medicare and Medicaid Services) regulations,
and payer-specific requirements.
- Participate in training on coding standards, federal regulations, and payer guidelines.
- Learn coding practices through mentorship from experienced team members.
- Documentation Review:
- Collaborate with the coding team to understand clinical documentation requirements.
- Alert senior coders to documentation gaps that may impact coding accuracy.
Qualifications:
- Education: Bachelor's degree in Health Information Management or related field preferred.
- Certification: Medical coding certification (CPC, CCS, or equivalent) required within 12 months of hire; or current enrollment in certification program.
- Experience:
- 1-4 years of medical coding experience.
- Coding internship or practicum experience is a plus.
- Knowledge:
- Working knowledge of medical terminology, basic anatomy and physiology.
- Understanding of ICD-10-CM, CPT, and HCPCS coding systems (foundational level).
- Familiarity with healthcare billing and reimbursement concepts.
- Basic proficiency with coding software and Electronic Health Records (EHR).
