JOB Description of a Resident Medical Officer

A Resident Medical Officer (RMO) serves as the primary clinical anchor on the floor in a standalone center relies entirely on the resident doctor to lead day-to-day operations and manage acute clinical emergencies. The resident doctor will act as the primary eyes and ears of the specialist( Nephrologist).

Core Job Responsibilities.

1. Pre-Dialysis Assessment

Before any patient is connected to a machine, the resident doctor must screen them to ensure it is protected to proceed:

- Vitals & Fluid Status: Assess pre-dialysis weight, blood pressure, heart rate, and check for signs of fluid overload (such as fluid in the lungs or pedal edema).
- Vascular Access Check: Examine the patient's arteriovenous (AV) fistula, graft, or central venous catheter for signs of infection, patency (checking for a proper "thrill" or bruit), or thrombosis. Docthub
- Reviewing Labs: Monitor monthly or periodic blood work (hemoglobin, potassium, calcium, phosphorus) and adjust the dialysis prescription parameters accordingly, keeping the nephrologist informed.

2. Intra-Dialysis Monitoring & Complication Management

Patients undergo hemodialysis for 3 to 4 hours, a period during which dramatic fluid shifts can cause acute complications. The resident doctor must be physically present on the floor to manage:

- Hypotension: The most common complication. The doctor must quickly manage sudden drops in blood pressure by administering IV normal saline, adjusting the ultrafiltration (UF) rate, or altering the patient's positioning.
- Muscle Cramps & Disequilibrium: Address severe cramping or Dialysis Disequilibrium Syndrome (headaches, nausea, blurred vision) by altering dialysate composition or administering hypertonic solutions.
- Vascular Dislodgement or Bleeding:



Immediately manage any accidental needle dislodgement or major bleeding from the access site.

3. Emergency & Code Blue Situations

As the lone physician on-site, the resident must be proficient in Advanced Cardiac Life Support (ACLS):

- Initiate cardiopulmonary resuscitation (CPR), intubate if necessary, and manage cardiac arrhythmias or sudden cardiac arrest.
- Stabilize highly critical patients and seamlessly coordinate their transfer via ambulance to the nearest tied-up multi-specialty hospital in Mumbai.

4. Quality Control & Infection Protocol

- Water Treatment Plant (WTP) Oversight: Hemodialysis requires highly purified water. The resident doctor, alongside senior technicians, monitors the WTP logs (Endotoxin levels, chemical testing, and reverse osmosis parameters) to ensure compliance with Indian Society of Nephrology (ISN) guidelines.
- Cross-Infection Prevention: Strictly enforce isolation protocols for patients who are positive for Hepatitis B, Hepatitis C, or HIV to avoid cross-contamination in the unit.
- NABH Standards: Maintain rigorous documentation, electronic medical records (EMR), and ensure the center adheres to National Accreditation Board for Hospitals & Healthcare Providers (NABH) guidelines. AlmaBay

5. Multidisciplinary Leadership & Counseling

- Team Leader: Supervise, train, and guide the dialysis technicians and nursing staff.
- Patient & Family Counseling: Guide patients through the psychological stress of chronic kidney disease (CKD), counsel them on strict fluid restrictions (usually 1 liter per day), and emphasize dietary modifications (low potassium, low sodium).
