Job Title: Home Care Nurse

Job Summary:
A Home Care Nurse provides professional nursing care to patients in their homes. They assess patients' health, administer medications and treatments, monitor recovery, educate patients and families, and coordinate with physicians to ensure high-quality, personalized care.

Key Responsibilities:

- Assess patients' physical and mental health conditions.
- Develop and implement individualized care plans.
- Administer medications, injections, IV therapy, and other prescribed treatments.
- Monitor vital signs and document patient progress.
- Perform wound care, catheter care, tracheostomy care, and other nursing procedures as required.
- Assist patients with activities of daily living (ADLs) when needed.
- Educate patients and caregivers on disease management, medication use, nutrition, and home safety.
- Identify changes in patient condition and report concerns to the physician.
- Maintain accurate and timely patient records.
- Ensure infection prevention and follow safety protocols.
- Provide emotional support to patients and their families.
- Coordinate with doctors, therapists,



and other healthcare professionals to ensure continuity of care.

Qualifications:

- Diploma, GNM, or B.Sc. Nursing.
- Valid nursing registration/license (as required by local regulations).
- Experience in home healthcare, hospital, or community nursing is an advantage.

- Solid clinical and patient assessment skills.
- Excellent communication and interpersonal abilities.
- Compassion, empathy, and patience.
- Ability to work independently and manage time effectively.
- Knowledge of infection control and patient safety practices.
- Basic computer and documentation skills.

Working Conditions:

- Travel to patients' homes.
- Flexible shifts, including evenings, weekends, and holidays if required.
- Ability to lift or assist patients safely when necessary.

Salary:
Salary depends on qualifications, experience, location, and employer policies.

Pay: ₹25,000.00 - ₹35,000.00 per month
