We are seeking a skilled and compassionate Hospice LVN to join our team. The LVN will be responsible for providing nursing care to patients in their homes, under the direction of a registered nurse or physician. The ideal candidate will have experience in hospice care, excellent communication skills, and a passion for helping others.
Responsibilities
- Assess patients' health status and develop care plans
- Administer medications and treatments as prescribed by a physician
- Monitor patients' vital signs and report any changes to the RN or physician
- Provide wound care and other nursing interventions as needed
- Document patient care and progress in electronic medical records
- Communicate with patients, families,
and other healthcare providers as needed
- Participate in interdisciplinary team meetings and case conferences
- Adhere to all state and federal regulations and agency policies and procedures
Requirements:
- Current LVN license in the state of Texas
- Minimum of one year of experience in hospice care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Robust critical thinking and problem-solving skills
- Valid driver's license and reliable transportation
- Ability to pass a background check and drug screening
