1. Patient Care Delivery
- Visit patients at their homes to provide high-quality nursing care as per the care plan and instructions of the Nursing Supervisor.
- Perform comprehensive patient assessments, including monitoring vital signs, medical history, physical condition, and overall health status.
- Administer prescribed medications, injections, IV fluids, wound care, oxygen therapy, nebulization, catheter care, feeding through Ryle's tube/PEG tube, and other treatments as ordered by the physician.
- Monitor patient progress and identify any changes in condition, reporting concerns promptly to the Nursing Supervisor or treating physician.
- Provide emergency first aid and basic life support (BLS) until advanced medical assistance is available, when required.
2. Personal Hygiene & Bedridden Patient Care
- Assist patients with activities of daily living (ADLs), including bathing, sponge baths, oral hygiene, grooming, shaving, hair care, nail care, dressing, and toileting.
- Provide complete personal hygiene care to bedridden patients while maintaining dignity, privacy, and comfort.
- Reposition bedridden patients at regular intervals to prevent pressure sores (bedsores) and maintain skin integrity.
- Perform skin assessments and pressure ulcer prevention measures.
- Assist with diaper changes, incontinence care, urinary catheter care, and bowel care while maintaining infection control practices.
- Assist with feeding, hydration, and nutritional support as per the prescribed dietary plan.
- Support patient mobility, including transfers, ambulation assistance, range-of-motion exercises, and fall prevention.
- Maintain clean bed linen, patient surroundings, and ensure a hygienic workplace.
3. Clinical Procedures
- Collect specimens for laboratory investigations as instructed.
- Monitor blood glucose levels,
blood pressure, pulse, temperature, oxygen saturation, respiratory rate, and pain levels.
- Perform wound dressing and post-operative care following aseptic techniques.
- Manage tracheostomy care, suctioning, ostomy care, and ventilator support (where applicable and trained).
- Ensure proper handling and maintenance of medical equipment used at the patient's home.
4. Patient & Family Education
- Educate patients and caregivers about medications, disease management, nutrition, hygiene, rehabilitation, and preventive care.
- Train family members in basic nursing care techniques where appropriate.
- Encourage patient independence while ensuring safety.
5. Care Plan Implementation & Collaboration
- Follow individualized care plans developed by the Nursing Supervisor and treating physician.
- Coordinate with doctors, physiotherapists, dietitians, caregivers, and other healthcare professionals to ensure comprehensive patient care.
- Participate in multidisciplinary discussions regarding patient progress and treatment plans.
6. Documentation & Reporting
- Maintain accurate and timely nursing records, including assessment findings, medications administered, treatments provided, and patient responses.
- Prepare daily nursing reports and communicate significant observations to the Nursing Supervisor.
- Report incidents, adverse events, medication errors, or patient safety concerns immediately.

Travel & Duty Management
- Travel to assigned patient homes within designated service areas.
- Maintain punctuality and adhere to scheduled visits, including 8-hour, 12-hour, or 24-hour duty shifts as assigned.
- Manage travel-related responsibilities professionally while ensuring timely patient care.
8. Infection Prevention & Safety
- Adhere to infection prevention and control protocols, including hand hygiene and proper use of Personal Protective Equipment (PPE).
- Dispose of biomedical waste according to applicable regulations.
- Maintain a safe environment for patients, caregivers, and self by following established safety protocols.
9. Professional Conduct
- Maintain patient confidentiality in accordance with healthcare regulations and organizational policies.
- Demonstrate empathy, compassion, professionalism, and respect toward patients and their families.
- Maintain professional appearance, grooming, and ethical conduct during duty.
- Ensure respectful communication with patients, caregivers, and healthcare team members.
10. Training & Professional Development
- Participate in mandatory clinical training, workshops, and continuing nursing education programs.
- Stay updated on current homecare nursing practices, infection control guidelines, and patient safety standards.
- Mentor junior nursing staff and support team learning when assigned.
11. Quality & Compliance
- Comply with all organizational policies, Standard Operating Procedures (SOPs), and applicable healthcare regulations.
- Ensure quality nursing care through adherence to evidence-based practices.
- Participate in quality improvement initiatives, patient satisfaction programs, and internal audits as required.
