Octagos Health is a US-headquartered, AI-powered cardiac remote patient monitoring SaaS platform serving cardiology clinics nationwide. We are scaling our engineering footprint in Noida to support the next phase of growth.

Role Summary
We are looking for an Advanced SQL Developer who lives and breathes T-SQL and Azure SQL performance. This role centers on four pillars: performance tuning at scale, database architecture for a multi-tenant healthcare platform, SQL code reviews that raise the engineering bar, and performance validation of every new database artifact before it ships. You will work directly with the Database Architect and Engineering leadership to drive reliability, scalability, and data quality.

Key Responsibilities
Performance Tuning
Tune complex T-SQL stored procedures, views, and ad-hoc queries against tables with hundreds of millions of rows.
Analyze execution plans, wait statistics, and Query Store data to identify and resolve CPU, IO, memory, and tempdb bottlenecks.
Design and refine indexing strategies including covering, filtered, columnstore, and partitioned indexes.
¢ Diagnose and remediate blocking, deadlocks, parameter sniffing, plan regressions, and statistics issues in production.
Database Architecture
¢ Contribute to the architecture of a multi-tenant Azure SQL platform using a database-per-tenant pattern with replicated reference data.
¢ Design schemas, partitioning strategies, and data lifecycle patterns for high-volume transactional workloads.
¢ Lead vertical decomposition, read/write separation, and CDC-based pipelines feeding reporting and analytics consumers.
¢ Evaluate Azure SQL features (Hyperscale, elastic pools, temporal tables, ledger tables, managed identity) and recommend adoption based on cost and performance trade-offs.
¢ Define standards for schema migrations, versioning,



and zero-downtime deployments.
Performance Monitoring and Load Testing
¢ Own the performance monitoring stack across Azure SQL using Database Watcher, Redgate Monitor, SolarWinds DPA, Query Store, and Extended Events.
¢ Establish baselines, dashboards, and alerting for query latency, throughput, deadlocks, blocking, and resource utilization.
¢ Build and maintain load testing harnesses (Azure Load Testing, JMeter, or equivalent) that exercise the data tier under realistic concurrency and data volumes.
¢ Gate every new stored procedure, view, index change, and schema migration with a documented load test and performance benchmark before production release.
¢ Publish before-and-after performance evidence for major changes, including plan comparisons, latency percentiles, and resource consumption deltas.
¢ Partner with engineering teams to integrate database performance gates into CI/CD pipelines.
Code Reviews and Best Practices
¢ Lead T-SQL code reviews across US and India engineering teams, enforcing standards for readability, parameterization, transaction scope, error handling, and security.
¢ Author and maintain SQL coding standards, including naming conventions, formatting, deprecation patterns, and anti-patterns to avoid.
¢ Mentor engineers on writing SET-based, plan-stable, index-friendly queries.
¢ Establish review checklists covering performance, HIPAA-aligned data handling, idempotency, and observability.
¢ Drive a culture where every stored procedure shipped is reviewed, benchmarked, instrumented, and load-tested.
Required Qualifications




¢ 7+ years of professional SQL Server / Azure SQL development experience.
¢ Expert-level T-SQL: window functions, CTEs, recursive queries, dynamic SQL, JSON handling, temporal tables.
¢ Proven track record of optimizing queries against very large, wide-column transactional tables.
¢ Deep knowledge of SQL Server internals: query optimizer, indexing, statistics, locking and isolation, tempdb, memory grants.
¢ Hands-on experience with Azure SQL Database in production at scale.
¢ Production experience with MySQL, including query tuning, indexing, and replication.
¢ Experience designing and executing database load tests and performance benchmarks.
¢ Familiarity with at least one enterprise monitoring tool (Database Watcher, Redgate Monitor, SolarWinds DPA, SQL Sentry, or equivalent).
¢ Experience leading SQL code reviews and authoring engineering standards.
¢ Strong written and verbal communication skills in English.
Preferred Qualifications
¢ Healthcare or HIPAA-regulated environment experience.
¢ Familiarity with .NET / C# data access patterns (Dapper, Entity Framework).
¢ Experience with Azure Load Testing, JMeter, k6, or similar tooling against database backends.
¢ Experience with CDC, replication, and ETL/ELT pipeline design.
¢ Exposure to Power BI semantic models and DAX optimization.
¢ Background in multi-tenant SaaS data architecture.
¢ Exposure to NoSQL databases (MongoDB, Cosmos DB, DynamoDB) and PostgreSQL.
¢ Data architecture supporting domain-driven design with event-based and pub/sub patterns (e.g., Azure Service Bus, Event Grid, Kafka).
¢ Microsoft certifications: DP-300, DP-203, or equivalent.
