August is building autonomous clinical AI.

Today, August shows up as a health companion: people use it to understand symptoms, lab reports, prescriptions, medications, and the confusing space between doctor visits. But the company we are building is much bigger than a companion. We are building AI systems that can take on increasingly complex clinical work: reasoning through symptoms, interpreting medical context, identifying risk, escalating when needed, and helping people get to the right next step.

Clinicians are at the heart of this.

We are looking for clinicians who want to work at the bleeding edge of healthcare: owning the clinical performance of AI systems, designing evaluation frameworks, reviewing model behavior, defining safety standards, and helping us deploy autonomous clinical use cases in the real world.

This is not a traditional clinical role. You will not just review outputs or write medical content. You will help shape how August thinks, reasons, escalates, explains, and improves. You will work closely with engineering, product, design, and AI teams to turn clinical judgment into measurable AI performance.

What you'll work on

Clinical performance of AI systems

Own how well August performs across real medical scenarios: symptom assessment, triage, lab interpretation, medication reasoning, care navigation, follow-up, and escalation.

Evaluation systems

Build clinical evals that test the AI across accuracy, safety, empathy, escalation quality, uncertainty handling, guideline adherence, and real-world usefulness.

Autonomous clinical use cases
Work with the team as we deploy increasingly autonomous AI workflows, from health companion experiences to more structured clinical decision-support and care-navigation systems.

Clinical reasoning and safety
Help define how August should reason through patient context, ask follow-up questions, identify red flags, communicate uncertainty, and decide when human or emergency escalation is required.

Model review and improvement
Review AI conversations, identify failure modes, create rubrics, annotate examples, build test sets, and work with engineers to improve model performance.

Clinical product judgment
Make sure August is not just medically correct in theory, but useful, safe, understandable, and trustworthy for real people in real situations.

Responsibilities

- Own clinical evaluation for one or more August AI use cases.
- Create high-quality clinical test cases, gold-standard answers, rubrics,



and benchmark datasets.
- Evaluate AI performance across real and simulated patient conversations.
- Identify clinical failure modes, safety risks, hallucinations, poor reasoning patterns, missed red flags, and weak escalation behavior.
- Work with AI engineers to improve prompts, retrieval systems, agent workflows, verifier layers, and model evaluation pipelines.
- Define clinical quality metrics for accuracy, safety, empathy, escalation, completeness, and usefulness.
- Help build processes for continuous AI monitoring, regression testing, red-teaming, and post-launch quality review.
- Translate medical guidelines, clinical workflows, and physician judgment into clear product and AI requirements.
- Partner with product and design teams to make AI responses clinically sound and easy for users to understand.
- Support deployment of autonomous clinical workflows with the right safety, review, and escalation systems.
- Build clinical policies for high-risk areas such as emergency symptoms, medication safety, pregnancy, pediatrics, chronic disease, mental health, and abnormal lab results.
- Help August decide when the AI should answer, ask more questions, escalate, refuse, or route to a human.

You might be a good fit if you have

- A clinical degree such as MBBS, MD, DO, or equivalent.
- Strong clinical reasoning and ability to think through ambiguous medical cases.
- Comfort reviewing patient conversations, medical histories, lab reports, prescriptions, and symptom presentations.
- Strong judgment around safety, uncertainty, escalation, and patient communication.
- Excitement about AI and its potential to transform healthcare access.
Ability to work with engineers and product teams, even if you do not code.
- Strong written communication. You can turn clinical thinking into clear rubrics, policies, examples, and product requirements.
- Bias toward ownership. You do not wait for perfect instructions; you help define the problem and drive it to completion.
- Comfort operating in unstructured, fast-moving environments.
- Deep care for patients, especially people navigating healthcare without easy access to doctors.

Nice to have

- Experience in primary care, emergency medicine, internal medicine, family medicine, pediatrics, psychiatry, or digital health.
- Experience with clinical audits, quality improvement,



medical safety, or protocol development.
- Experience reviewing or building clinical decision-support systems.
- Experience with AI evaluation, model testing, annotation, red-teaming, or prompt evaluation.
- Familiarity with LLMs, RAG, agents, model behavior, or AI safety.
- Experience working with health-tech products, telemedicine, digital triage, or care navigation.
- Comfort with tools like spreadsheets, annotation platforms, basic SQL, Python, or no-code evaluation tools.
- Experience creating clinical guidelines, care pathways, SOPs, or medical content for patient-facing products.

You will be part of the team defining how autonomous clinical AI should behave: what it should know, how it should reason, when it should be cautious, how it should communicate, and how its performance should be measured.
