Job Summary
The Clinical Pharmacist is responsible for optimizing medication therapy and promoting safe, effective, and evidence-based use of medicines across various clinical specialties. The role involves participating in multidisciplinary patient care, conducting medication reviews, providing drug information, monitoring adverse drug reactions, and supporting antimicrobial stewardship while ensuring compliance with NABH standards and hospital policies.
Key Responsibilities
- Review medication orders for appropriateness, dosage, interactions, contraindications, and therapeutic duplications.
- Participate in multidisciplinary ward rounds and collaborate with consultants, nurses, and other healthcare professionals.
- Monitor patients for medication effectiveness, adverse drug reactions (ADRs), and medication errors.
- Document and report ADRs and medication errors as per hospital protocols.
- Provide drug information and evidence-based recommendations to healthcare professionals.
- Counsel patients and caregivers on medication usage, storage, adherence, and possible side effects.
- Participate in medication reconciliation during admission, transfer, and discharge.
- Support Antimicrobial Stewardship (AMS) initiatives by reviewing antibiotic prescriptions and promoting rational antibiotic use.
- Assist in developing and updating clinical guidelines, drug policies, and standard treatment protocols.
- Conduct prescription audits and contribute to quality improvement initiatives.
- Maintain accurate clinical documentation and intervention records.
- Participate in clinical research, pharmacovigilance, and educational activities.
- Ensure compliance with NABH, NABL, and statutory requirements related to medication management.
- Coordinate with the Pharmacy Department to ensure the availability and appropriate utilization of medicines.
- Participate in continuous professional development and departmental training programs.
Required Skills
- Strong knowledge of pharmacotherapy and clinical pharmacy practices.
- Sound understanding of pharmacokinetics, pharmacodynamics, and evidence-based medicine.
- Knowledge of medication safety, pharmacovigilance, and antimicrobial stewardship.
- Excellent clinical judgment and analytical skills.
- Strong communication and patient counselling skills.
- Proficiency in Microsoft Office and Hospital Information Systems (HIS/EMR).
- Ability to interpret laboratory values and correlate them with medication therapy.
- Good documentation and presentation skills.
Preferred Competencies
- Experience in a multi-specialty tertiary care hospital.
- Familiarity with NABH standards and quality improvement initiatives.
- Certification or training in Clinical Pharmacy, Pharmacovigilance, or Antimicrobial Stewardship is an added advantage.
- Strong interpersonal skills with the ability to work effectively in multidisciplinary teams.
- Commitment to continuous learning and professional development.

Pay: ₹12,940.69 - ₹25,000.00 per month
