Location: 100-Bedded Multispecialty Hospital
Reports To: Pharmacy Manager / Head of Pharmacy

Position Summary

The Pharmacist is responsible for ensuring the safe, accurate, and timely dispensing of medications to inpatients and outpatients while maintaining compliance with statutory regulations, hospital policies, NABH standards, and patient safety protocols. The role requires close collaboration with doctors, nurses, and other healthcare professionals to deliver high-quality pharmaceutical care.

Key Responsibilities

Medication Dispensing & Patient Care

- Dispense prescribed medications accurately after verifying prescriptions.
- Review prescriptions for dosage, drug interactions, allergies, contraindications, and completeness.
- Counsel patients regarding medication usage, dosage, storage, precautions, and possible side effects.
- Ensure proper labeling and documentation of all dispensed medicines.
- Coordinate with clinicians to clarify medication orders whenever required.

Inventory & Store Management

- Monitor medicine inventory and maintain adequate stock levels.
- Ensure proper storage of medicines as per manufacturer's recommendations.
- Monitor expiry dates and remove near-expiry or expired medications promptly.
- Maintain narcotic, psychotropic, and high-alert medication registers as per statutory requirements.
- Participate in procurement planning and stock reconciliation.

Clinical & Hospital Support

- Support medication reconciliation during patient admission and discharge.
- Participate in ward rounds when required.
- Report adverse drug reactions (ADR)



and medication errors.
- Assist in implementing antimicrobial stewardship and medication safety initiatives.
- Support code blue and emergency medication management.

Quality & Compliance

- Ensure compliance with NABH, Pharmacy Act, Drugs & Cosmetics Act, and hospital policies.
- Maintain accurate pharmacy documentation and records.
- Participate in internal audits and quality improvement initiatives.
- Follow infection control and biomedical waste management protocols.
- Ensure confidentiality of patient information.

Documentation

- Maintain dispensing records.
- Record controlled drug transactions accurately.
- Prepare daily, weekly, and monthly pharmacy reports.
- Update inventory management systems regularly.

Qualifications

- Diploma in Pharmacy (D.Pharm) or Bachelor of Pharmacy (B.Pharm).
- Valid State Pharmacy Council Registration (Mandatory).

Experience

- 25 years of experience in a multispecialty hospital pharmacy.
- Experience in inpatient, outpatient, ICU, OT, and emergency pharmacy preferred.
- Knowledge of Hospital Information Systems (HIS) and pharmacy management software is desirable.

Required Skills

- Solid knowledge of pharmacology and medications.




- Knowledge of NABH standards and hospital pharmacy practices.
- Excellent attention to detail and accuracy.
- Good communication and patient counseling skills.
- Ability to work effectively with multidisciplinary teams.
- Computer proficiency and familiarity with pharmacy software.
- Good inventory and documentation skills.

Working Conditions

- Rotational shifts, including night shifts, weekends, and public holidays.
- May be required to work in Inpatient Pharmacy, Outpatient Pharmacy, Emergency Pharmacy, ICU Pharmacy, and OT Pharmacy based on operational requirements.

- Positive patient and clinician feedback

Preferred Candidate Profile

- Experience in a NABH-accredited hospital.
- Familiarity with barcode medication administration systems.
- Certification in medication safety or clinical pharmacy will be an added advantage.
- Willingness to work in a fast-paced, patient-centric environment.
