CCDC also holds recognition as a WHO Collaborating Centre for Surveillance, Capacity Building and Translational Research in Cardio-metabolic diseases and is an Indian Council of Medical Research (ICMR) Collaborating Centre of Excellence.

Project Brief: CCDC is seeking a proactive and experienced project nurse to conduct patient related activities for a randomized multi-centric clinical trial. This project aims to evaluate the impact and feasibility of using a multicomponent digital health model for home-based care of post-discharge heart failure patients in India.

The Project Nurse will be responsible for patient-related activities and contribute to the implementation of this research study in the following manner:

- Assist in screening and recruitment for patients in the study
- Conduct routine home visits to enrolled patients as per the study protocol.
- Perform comprehensive patient history taking, physical, POC diagnostics testing and mental health assessments as per the study protocol.




- Facilitate effective communication and discussions between doctors and patients.
- Provide after-care counselling and patient education in line with clinical guidance.
- Follow standard operating procedures and health assessment checklists to ensure quality of care and study consistency.
- Create, maintain, and update patient health records in accordance with study requirements.
- Conduct scheduled follow-up visits and record clinical and study related data per study protocol.
- Prepare administrative reports, documentation, and periodic study updates as required.
- Support the study team in monitoring patient progress and adherence to care plans.
- Ensure ethical conduct, confidentiality, and data accuracy during all patient interactions.
- Attend and participate in stakeholders’ meetings, and other meetings as needed.
- Maintain high-quality data collection, management, and storage, in collaboration with data teams.
- Report on a regular basis to the project team regarding project and seek timely advice wherever necessary.
- Any other task assigned by the supervisor/ study PI.

Responsibilities And Requirements

- Education: Three-year General Nursing & Midwife (GNM)



course (All the educational qualifications/certificates shall be from the recognized Board/University).
- 1-2 years of work experience in large implementation research projects.
- Experience of working in large national and state-level programs preferred (Experience from reputed National/International Organizations shall also be considered)
- Experience gained shall be counted after the date of completion of Minimum Essential Qualification.
- Proven ability to work in a diverse team.
- Excellent communication and interpersonal skills with ability to read, write and speak in English and regional language.
