Key Responsibilities
Clinical & Medication Management
- Provide clinical pharmacology support to physicians and multidisciplinary clinical teams.
- Review medication regimens for appropriate drug selection, dose, frequency, route, and duration.
- Identify potential drug–drug, drug–food, and drug–disease interactions.
- Support appropriate medication selection based on patient-specific factors such as age, renal function, hepatic function, allergies, and comorbidities.
- Participate in clinical rounds, particularly in critical care and high-risk patient areas, where required.
- Provide evidence-based recommendations regarding medication therapy.
- Assist clinicians in optimizing medication regimens for complex and high-risk patients.
- Support therapeutic drug monitoring for medicines where clinically indicated.
- Provide guidance on dose adjustments in renal and hepatic impairment.
- Assist in medication reconciliation during patient admission, transfer, and discharge.
Adverse Drug Reaction & Medication Safety
- Establish and monitor systems for identification, documentation, reporting, and prevention of adverse drug reactions (ADRs).
- Evaluate suspected ADRs and provide recommendations to the treating team.
- Maintain appropriate records of ADRs and medication-related incidents.
- Identify medication errors and near misses and recommend preventive measures.
- Participate in medication-safety audits and root-cause analysis of medication-related incidents.
- Monitor high-alert and high-risk medications and support implementation of appropriate safety measures.
- Assist in developing medication safety alerts and educational communications for healthcare professionals.
Antimicrobial Stewardship
- Support the hospital's Antimicrobial Stewardship Program.
- Review antimicrobial prescriptions for appropriate indication, selection, dose, route, and duration.
- Work with clinicians and microbiology teams to promote rational antimicrobial use.
- Support antimicrobial de-escalation, IV-to-oral conversion, and appropriate duration of therapy where clinically indicated.
- Monitor antimicrobial utilization and identify opportunities for improvement.
- Participate in antimicrobial stewardship meetings, audits,
and educational initiatives.
- Assist in developing and updating antimicrobial guidelines based on hospital antibiogram and current evidence.
Drug Information & Clinical Support
- Provide accurate, evidence-based drug information to doctors, nurses, pharmacists, and other healthcare professionals.
- Respond to medication-related queries and provide documented recommendations when required.
- Review new medicines, formulations, and therapeutic alternatives for potential inclusion in the hospital formulary.
- Assist in preparing drug information bulletins, medication alerts, and clinical updates.
- Support clinicians with evidence-based information regarding dosage, contraindications, precautions, interactions, and monitoring requirements.
Formulary & Policy Development
- Participate in Pharmacy & Therapeutics Committee activities.
- Assist in development, review, and implementation of medication-use policies and protocols.
- Support periodic review of the hospital formulary.
- Evaluate medication utilization and identify areas for rationalization.
- Assist in developing guidelines for high-risk medications, antimicrobial use, medication reconciliation, and other medication-safety initiatives.
- Support standardization of medication practices across clinical departments.
Training & Education
- Conduct educational sessions for doctors, nurses, pharmacists, and other healthcare professionals on medication-related topics.
- Provide training on safe medication practices, high-alert medications, ADR reporting, antimicrobial stewardship, and drug interactions.
- Participate in continuing medical education (CME), workshops, and clinical case discussions.
- Promote a culture of rational and evidence-based medicine within the hospital.
Quality & Research
- Participate in clinical audits, quality-improvement projects,
and medication-use evaluations.
- Support hospital accreditation and quality initiatives related to medication management and patient safety.
- Participate in clinical research, pharmacovigilance activities, and academic projects where applicable.
- Maintain appropriate documentation and reports related to clinical pharmacology activities.
Coordination & Compliance
- Coordinate with Pharmacy, Nursing, ICU, OT, Emergency, Laboratory, Microbiology, Infection Control, and clinical departments.
- Maintain confidentiality of patient and hospital information.
- Ensure compliance with hospital policies, medication-management standards, and applicable regulatory requirements.
- Assist during internal and external audits related to medication management.
- Escalate significant medication-safety concerns to the appropriate clinical and administrative authority.
Qualification
- MD/DNB in Clinical Pharmacology from a recognized institution.
- Candidates with relevant postgraduate qualifications and substantial hospital-based clinical pharmacology experience may be considered, subject to hospital requirements and applicable regulations.
Experience
- 1–5 years of relevant experience in a hospital/healthcare setting preferred.
- Experience in medication safety, pharmacovigilance, antimicrobial stewardship, ICU/critical care support, or clinical pharmacy services will be an advantage.
- Fresh postgraduate candidates with strong clinical exposure may also be considered.

Pay: ₹30,000.00 - ₹55,000.00 per month
