- Industrial Research (DSIR), Ministry of Science and Technology, Government of India. CCDC also holds recognition as a WHO Collaborating Centre for Surveillance, Capacity Building and Translational Research in Cardio-metabolic diseases and is an ICMR Collaborating Centre of Excellence.

Project Brief: The CHANGE–CVD (Reconceptualising Primary Health System for Cardiovascular Care using a Multicomponent Digital Health Model) initiative is an 18-month implementation research project aimed at transforming cardiovascular disease (CVD) care at the primary healthcare level in India. The project combines digital health technologies, point-of-care diagnostics, telemedicine, and clinical decision support systems to improve early detection, risk assessment, treatment, and referral of heart diseases. Implemented across ten Rural Health Training Centres (RHTCs) linked to five leading medical colleges, the initiative seeks to build a scalable, technology-enabled model for delivering accessible, standardised, and patient-centred cardiovascular care in rural and underserved communities.

The Staff Nurse will contribute in the following manner:

Clinic &

- Patient Management

- Screen the patients with cardiovascular diseases, hypertension, diabetes and related risk factors.





- Conduct assessment of the patients using the digital health platform and record data.

- Coordinate with the medical officers, RHTC/UHC/PHC staff and specialists to ensure smooth implementation of the intervention.

- Assist medical offers and specialists in conducting online and offline consultations.

Program Coordination &

- Stakeholder Engagement

- Serve as the primary point of contact for ensuring effective communication with PHC/RHTC stakeholders.

- Implement digital health strategies aligned with the project objectives in collaboration with relevant stakeholders.

- Prepare and disseminate reports regarding the activities and milestones of the project at the respective site.

- Coordinate screening activities of patients and facilitate all other project activities as required by the research study protocol.

Field Implementation &

- Monitoring

- Identify operational bottlenecks and coordinate solutions for efficient project delivery.

Reporting &

- Administrative Support

- Provide regular updates to the project investigators, ensuring transparency in project execution and seeking guidance when necessary.

- Perform any additional responsibilities as assigned by the Supervisor or Project Principal Investigator (PI).

Monitoring, Evaluation &

- Reporting

- Conduct regular monitoring of project activities.

- Prepare, document, and submit regular reports (weekly, monthly) to the central team at CCDC.

Any Other Tasks

- Perform any other tasks as assigned by the Supervisor to support the successful implementation of the project.

- Regularly check and monitor the project requirements.





- Coordinate with other project staff and other local stakeholders to ensure smooth implementation of project activities.

- Attend and participate in meetings as needed.

Essential

Responsibilities and Requirements

- Post Basic Bachelor of Sciences in Nursing (PB B.Sc. Nursing) or General Nursing and Midwifery (GNM) or Auxiliary Nursing and Midwifery (ANM)

- Experience (1-2 years) or fresher

- Excellent communication and interpersonal abilities.

- Proficiency in local languages as applicable.

- Working knowledge of Ms Word, Excel, Zoom platform, Android applications and other digital applications.

Desirable

- Experience of working in a health facility setting.

- Ability to work collaboratively with diverse teams and stakeholders.

- Strong problem-solving skills and attention to detail.

Reporting The Staff Nurse will report to the assigned project leadership from the central CCDC team and work closely with the stakeholders at the study site.
