Job Summary:
The Palliative Care Coordinator is responsible for coordinating and managing palliative care services to ensure holistic, patient-centered care for individuals with life-limiting illnesses. The role involves coordinating multidisciplinary teams, facilitating home care and inpatient services, maintaining patient records, liaating with families, and ensuring smooth implementation of palliative care programs.
Key Responsibilities:
Patient Care Coordination
- Coordinate admission, assessment, and follow-up of palliative care patients.
- Develop individualized care plans in collaboration with the multidisciplinary team.
- Schedule and coordinate home visits, clinic appointments, and inpatient consultations.
- Monitor patient progress and ensure continuity of care.
Family Support
- Educate patients and caregivers regarding disease management and available support services.
- Provide emotional support and guidance to patients and their families.
- Coordinate bereavement support services when required.
Team Coordination
- Coordinate activities of doctors, nurses, social workers, psychologists, physiotherapists, volunteers, and other allied healthcare professionals.
- Organize multidisciplinary case discussions and care review meetings.
- Ensure timely communication among all stakeholders.
Program Management
- Maintain patient databases and documentation accurately.
- Prepare monthly activity reports, patient statistics, and service utilization reports.
- Monitor service quality and recommend improvements.
- Ensure compliance with organizational policies and palliative care guidelines.
Community Outreach
- Coordinate community awareness programs on palliative care.
- Build relationships with hospitals, NGOs, community organizations, and referral partners.
- Support volunteer recruitment, orientation, and scheduling.
Administrative Responsibilities
- Maintain patient records while ensuring confidentiality.
- Coordinate procurement and availability of medical supplies and equipment required for patient care.
- Assist in budgeting and resource planning for palliative care activities.
- Ensure proper documentation for government and donor reporting requirements
Requirements
Qualifications
- Bachelor's or Master's Degree in Social Work (BSW/MSW), Nursing (B.Sc./GNM), Psychology, Healthcare Administration, or any allied health sciences.
- Certification or training in Palliative Care is preferred.
Experience
- Minimum 0–1years of experience in palliative care, hospice care, community health, or healthcare coordination.
- Experience working with multidisciplinary healthcare teams is preferred
