Position Summary
At Stem Health, we believe that exceptional care does not end at the appointment; it extends through every referral, every follow-up, and every moment a member is navigating a system that can feel complex and impersonal. Our Care Coordinator and Client Advocate is the person who makes that navigation feel effortless.
We are seeking a compassionate, clinically grounded,
and highly organized individual to serve as the connective tissue between our members and the broader healthcare system. In this role, you will manage the full arc of care coordination from specialist referrals and diagnostic scheduling to care transitions and follow-through while building the kind of trusted, ongoing relationship with each member that is central to everything we do. This role is ideal for someone with a clinical or health navigation background who leads with advocacy, excels at follow-through, and believes that nothing should fall through the cracks.
Key Responsibilities
Care coordination and referral management
Act as the primary liaison between members, specialists, hospitals, diagnostic labs, and community providers ensuring care transitions are smooth, timely, and well-documented.
Initiate, track, and follow up on specialist referrals, diagnostic orders, and clinical procedures from start to completion, ensuring no step is missed and
