E/M Surgery Medical Coder (Orthopedic, Neurology, Cardiology , IVR)

Job Summary:

An E/M (Evaluation and Management) Surgery Medical Coder is responsible for reviewing physician documentation and assigning accurate CPT, ICD-10-CM, HCPCS, and modifier codes for surgical E/M services. The coder ensures compliance with AMA, CMS, and payer-specific guidelines while supporting accurate reimbursement and reducing claim denials.

Key Responsibilities

- Review and analyze surgical patient records, office visits, consultations, hospital visits, and follow-up encounters.
- Assign accurate E/M CPT codes based on Medical Decision Making (MDM) and time-based coding guidelines.
- Apply appropriate ICD-10-CM diagnosis codes, HCPCS codes, and modifiers.
- Code E/M services for surgical specialties such as:

- General Surgery

- Orthopedic Surgery

- Neurosurgery

- ENT Surgery

- Urology

- Cardiothoracic Surgery

- Plastic Surgery
- Ensure documentation supports the selected E/M level.
- Identify and report documentation deficiencies to providers when clarification is required.
- Follow current AMA CPT and CMS E/M coding guidelines.
- Ensure compliance with Medicare, Medicaid, HIPAA, and commercial payer requirements.
- Review denied claims and assist in coding corrections when necessary.
- Meet productivity and quality standards established by the organization.




- Stay updated on annual coding guideline changes.

Required Qualifications

- Bachelor's degree or equivalent in Life Sciences, Pharmacy, Nursing, or related healthcare field.

- Certification preferred

- CPC (Certified Professional Coder)

- CCS (Certified Coding Specialist)

- COC (Certified Outpatient Coder)

Required Skills

- Solid knowledge of E/M coding guidelines.
- Knowledge of CPT, ICD-10-CM, HCPCS, and modifiers.
- Understanding of surgical documentation and medical terminology.
- Excellent analytical and problem-solving skills.
- Attention to detail and coding accuracy.
- Familiarity with EMR/EHR systems.
