Role & responsibilities

- Review and validate prescriptions to ensure protected and appropriate medication use.
- Monitor patients' medication therapy and recommend necessary adjustments.
- Identify and prevent drug interactions, adverse drug reactions, and medication errors.
- Provide medication-related information and support to doctors, nurses, and other healthcare professionals.
- Counsel patients on medication usage, dosage, side effects, and adherence.
- Participate in clinical rounds and contribute to patient care decisions.
- Monitor and document adverse drug reactions (ADRs) and medication incidents.
- Ensure compliance with hospital policies, regulatory requirements, and medication safety standards.
- Maintain accurate clinical pharmacy records and reports.
- Conduct medication audits, drug utilization reviews, and educational programs for healthcare staff.
