Job Title: Medical Coder
Department: Medical Records Department (MRD)
Experience: 0-1 years preferred
Qualification: Graduate in Life Sciences / Allied Health / Medical Coding certification preferred

Key Responsibilities
- Review patient medical records, discharge summaries, clinical notes, investigation reports, and treatment documentation.
- Assign accurate ICD and other applicable diagnosis/procedure codes based on clinical documentation.
- Ensure completeness and accuracy of medical records before coding.
- Identify discrepancies, missing information, or unclear documentation and coordinate with relevant departments for clarification.
- Maintain confidentiality and security of patient medical information.
- Follow hospital policies, coding guidelines, and applicable regulatory requirements.
- Support MRD audits and provide required coding-related documentation.
- Maintain accurate coding records and assist in preparing departmental reports.
- Coordinate with Medical Records, Billing,
Clinical, and Insurance/TPA departments when required.
- Ensure timely completion of coding activities within defined turnaround time.
- Participate in quality improvement activities and coding accuracy reviews.
- Stay updated with changes in coding guidelines, healthcare documentation requirements, and relevant standards.
Skills & Competencies
- Good knowledge of medical terminology and anatomy.
- Knowledge of ICD coding and medical documentation.
- Solid attention to detail and accuracy.
- Good analytical and interpretation skills.
- Basic computer and MS Office skills.
- Good communication and coordination skills.
- Ability to maintain strict confidentiality of patient information.
- Ability to work within deadlines and manage multiple records.
Pay: ₹10,000.00 - ₹25,000.00 per month
