Role Overview

Our client is seeking a Cardiology Patient Navigator (Coordinator) to support a busy cardiology care team overnight and on weekends. The role centers on patient communication, scheduling, care coordination, remote monitoring alert triage, and clinical documentation. The ideal candidate brings a clinical background, an active nursing license (RN or equivalent),



experience in telephonic or remote care settings, and comfort managing a high-volume patient caseload across multiple priorities.

Cardiology-specific experience is preferred but not required. Candidates with strong backgrounds in care coordination, care management, med-surg, ICU, cardiac step-down, CCU, or home health will be competitive. What matters most is clinical credibility, robust communication, and the ability to keep patients connected to their care plan.

Key Responsibilities

Patient Communication & Scheduling
- Manage patient scheduling, reminders, and follow-up workflows in EMR/EHR systems
- Conduct telephonic or remote patient outreach to reinforce care instructions and coordinate next steps
- Support patients navigating cardiology care pathways, including CHF management protocols

Remote Monitoring & Care Coordination
- Monitor remote patient monitoring alerts for patient weight and blood pressure readings
- Escalate abnormal readin
