- Engagement with Identified Beneficiaries: Build trust and maintain consistent communication with individuals in need of assistance, especially those experiencing homelessness or living in at-risk environments.
- Field Visits & Mapping: Conduct regular outreach visits to identify, assess, and document locations where beneficiaries reside or frequent, creating accurate maps for service delivery.
- Assistance in Hospitalisation and Street Medical Care: Support clients in accessing emergency and routine medical care, including accompanying them to hospitals and coordinating with street medical teams.
- Promoting Hygiene and Sanitation: Ensure basic hygiene standards are maintained among the beneficiaries through education, provision of hygiene kits, and monitoring.
- Community Care Giver Development: Identify, develop, and coordinate a network of local community care givers to extend the reach of care and support.
- Follow-Up and Active Tracking: Monitor ongoing cases by maintaining regular contact, tracking progress, and ensuring continuity of care and services.
