Job Overview

Frontpoint Health is seeking compassionate and detail-oriented Registered Nurses (RNs) to join our US Operations team in a remote, full-time role. This position primarily involves participating in and coordinating case conference activities to support patient care planning and interdisciplinary collaboration in a home healthcare setting.

Key Responsibilities
- Participate in interdisciplinary case conferences and contribute clinical insights.
- Review patient records and assist in developing and updating care plans.
- Collaborate with physicians, case managers, therapists, and other healthcare professionals to ensure continuity of care.
- Document case conference discussions accurately and maintain patient records.
- Coordinate with internal teams to support timely patient care activities.
- Communicate effectively with healthcare providers and team members regarding patient status and care plans.
- Promote high-quality patient care through effective care coordination and documentation.
- Support additional clinical coordination tasks as assigned.

Qualifications




- Bachelor's Degree, Diploma, or equivalent qualification in Nursing.
- Active Registered Nurse (RN) license in India.
- Experience in home healthcare, care coordination, utilization review, or case management is an advantage.
- Strong clinical judgment, critical thinking, and documentation skills.
- Excellent written and verbal English communication skills.
- Comfortable working remotely during US shift hours.
- Reliable laptop and stable high-speed internet connection.

Preferred Skills
- Experience working with US healthcare processes is an added advantage.
- Ability to work collaboratively within a multidisciplinary healthcare team.
- Robust organizational and time management skills.
