Job Summary

We are looking for an experienced Denial Coder – E&M; to review, analyze, and resolve coding-related claim denials. The candidate will be responsible for identifying the root cause of denials, correcting coding errors, and ensuring accurate E&M; coding in accordance with ICD-10-CM, CPT, and payer guidelines.

Key Responsibilities

- Review denied claims related to E&M; (Evaluation & Management) coding.
- Analyze denial reasons and identify coding, documentation, and billing errors.
- Review medical records and verify E&M; codes against clinical documentation.
- Identify incorrect CPT, ICD-10-CM, modifiers, and diagnosis coding.
- Correct coding errors and recommend appropriate claim corrections.
- Work on E&M-related; denials, undercoding, overcoding, bundling, modifier, and medical necessity issues.
- Ensure coding compliance with payer-specific guidelines and coding regulations.




- Prepare and submit corrected claims or provide appropriate denial resolution recommendations.
- Maintain accuracy and productivity targets while meeting turnaround time requirements.
- Coordinate with coding, billing, AR, and quality teams to resolve recurring denial issues.
- Track denial trends and identify recurring coding-related issues.
- Maintain proper documentation of denial analysis and resolution.
- Stay updated with changes in CPT, ICD-10-CM, E&M; guidelines, and payer policies.

Pay: ₹20,000.00 - ₹35,000.00 per month
