Assist clinical pharmacists in reviewing patient medication profiles.

Participate in medication history taking and medication reconciliation.

Monitor drug therapy, dosage, interactions, and possible adverse drug reactions.

Assist in identifying and reporting drug-related problems.

Provide basic medication-related information to patients under supervision.

Participate in ward rounds and clinical discussions when permitted.

Support pharmacists in maintaining accurate medication records.

Assist in reviewing prescriptions for appropriate drug use.

Follow hospital policies, pharmacy procedures, and patient-safety standards.

Maintain confidentiality of patient information.

Participate in pharmacy-related audits, case discussions, and educational activities.

Perform other duties assigned by the clinical pharmacist or department supervisor.
