Job Title: Medical Coder - HCC

Location: Hyderabad

Job Summary

We are seeking experienced HCC Coders to join our team in Hyderabad. The ideal candidate will have a robust foundation in medical coding, excellent attention to detail, and a commitment to coding accuracy, compliance, and continuous improvement.

Responsibilities
- Review medical records to identify and validate patient diagnoses, conditions, and treatments.
- Assign appropriate ICD-10-CM diagnosis codes based on clinical documentation.
- Map documented diagnoses to Hierarchical Condition Categories (HCC).
- Ensure coding accuracy, consistency, and compliance with applicable coding guidelines.
- Review and audit medical records and documentation for coding accuracy and compliance.
- Identify documentation gaps and provide appropriate feedback to physicians and healthcare providers.
- Support clinical documentation improvement initiatives.
- Educate and guide team members on clinical documentation, coding guidelines, and HCC requirements.




- Maintain productivity and quality standards while meeting established turnaround times.
- Stay updated on changes to coding guidelines, HCC models, and healthcare regulations.

Requirements:
- 6 months to 4 years of relevant medical coding experience.
- Experience in HCC coding / Risk Adjustment coding is preferred.
- HCC Coding Certification is preferred but not mandatory.
- Candidates with a strong medical coding background and willingness to learn HCC coding will also be considered.
- Strong knowledge and practical experience in medical coding.
- Good understanding of ICD-10-CM, CPT, HCPCS, and HEDIS Category II codes.
- Knowledge of HCC coding and risk adjustment concepts.
- Strong attention to detail and ability to interpret clinical documentation accurately.
- Ability to identify coding discrepancies and documentation gaps.
- Strong analytical, communication, and problem-solving skills.
- Commitment to compliance, quality, accuracy, and continuous improvement.
