Job Overview

365 Medicare is looking for compassionate, clinically competent, and responsible Staff Nurses to provide professional nursing care to patients in their homes.

The nurse will be responsible for delivering safe patient care, monitoring clinical conditions, following the prescribed care plan, maintaining accurate documentation, educating family members/caregivers, and promptly escalating any change in the patient's condition.

Key Responsibilities
- Provide skilled nursing care to patients at home.
- Monitor and record vital signs and clinical observations.
- Administer medications and treatments as prescribed.
- Provide wound dressing and wound care.
- Perform catheter care and NG tube care as required.
- Provide tracheostomy care and suctioning where appropriately trained.
- Assist with oxygen therapy and other prescribed respiratory support.
- Monitor nutrition, hydration, elimination, mobility, and general patient condition.
- Assist bedridden and dependent patients with activities of daily living.




- Support post-operative and post-hospitalization recovery.
- Identify changes in the patient's condition and escalate appropriately.
- Follow the patient's individualized care plan.
- Maintain accurate nursing notes and daily care records.
- Communicate relevant clinical updates to the Care Manager and treating/referring team as required.
- Educate family members and caregivers regarding basic patient care and safety.
- Follow infection-control, medication-safety, patient-safety, and other 365 Medicare clinical protocols.
- Maintain professional conduct, patient dignity, confidentiality, and respect at all times.
- Coordinate with physiotherapists, doctors, care managers, and other healthcare professionals when required.

Pay: ₹12,699.68 - ₹48,649.44 per month
