KEY RESPONSIBILITIES:
- Transcribe medical reports, doctors' notes, discharge summaries, consultation notes, operative/procedure notes, progress notes, and other clinical documentation.
- Accurately interpret medical terminology, abbreviations, drug names, diagnoses, investigations, and treatment information.
- Proofread and edit transcribed documents for spelling, grammar, punctuation, medical terminology, and formatting.
- Maintain high standards of accuracy while transcribing patient-related information.
- Verify unclear or incomplete information with the concerned doctor or authorized medical staff when required.
- Ensure timely completion and submission of transcription work.
- Maintain confidentiality and privacy of patient information at all times.
- Follow hospital policies and procedures related to medical records and documentation.
- Enter and update medical reports in the Hospital Information System (HIS/HMIS/EMR)



or other approved systems.
- Maintain proper organization and filing of completed medical documents.
- Coordinate with doctors, nurses, medical records staff, and other departments regarding documentation requirements.
- Identify and report discrepancies, missing information, or documentation errors to the appropriate authority.
- Ensure that reports are prepared in the prescribed hospital format.
- Handle electronic and/or audio files containing confidential patient information securely.
- Maintain appropriate records of completed, pending, and corrected transcription work.
- Assist the Medical Records Department in other documentation-related activities as assigned.

Pay: ₹18,000.00 - ₹25,000.00 per month
