Job Title Consultant Home Care Nurse Department Home Health Care Services Location Insert Location Reports To Clinical Director Home Care Manager Employment Type Consultant Part-Time Per Visit Basis Job Summary The Consultant Home Care Nurse is responsible for providing professional nursing services to patients in their homes The role includes assessment planning implementation and evaluation of patient care The nurse will work collaboratively with physicians family members and other healthcare professionals to ensure high-quality patient-centered care Key Responsibilities Conduct in-home assessments of patients and document clinical status Develop and implement individual care plans based on patient needs Administer medications wound care injections IV therapy catheter care etc Educate patients and caregivers on disease management medication adherence and hygiene Monitor and document patient progress and update care plans as needed Coordinate with physicians and healthcare teams to ensure continuity of care Maintain accurate clinical records and reports in compliance with company and legal standards Provide emotional support and counseling to patients and families Follow infection control and patient safety protocols Respond to medical emergencies during home visits if required Qualifications Education B Sc Nursing GNM from a recognized institute License Registered Nurse with State Nursing Council Experience Minimum 1-2 years of clinical experience preferably in home healthcare ICU or general medical-surgical care Skills Strong clinical and assessment skills Good communication and interpersonal skills Ability to work independently Basic documentation and tech skills using tablets or apps for reporting Working Conditions Adaptable schedule based on visits or appointments Travel within assigned areas for home visits May involve weekend or evening shifts Remuneration Per visit hourly package-based as per company policy or agreement Job Types Full-time Permanent Pay 10 191 15 - 44 015 94 per month Perks Cell phone reimbursement Flexible schedule Food provided Life insurance Provident Fund Schedule Day shift Fixed shift Morning shift Supplemental Pay Commission pay Overtime pay Shift allowance Work Location In person
