Job Purpose

Transcribedictated reports and translate medical jargon and abbreviations into theirexpanded forms. Edit as necessary and return reports in either printed orelectronic form to the dictator for review and signature, or correction. Makedischarge summary of normal discharged, DAMA & death patients.

Essential duties and responsibilities
Co-ordinatewith medical officer for discharge summary of patient and prepare thecomputerized discharge summary from the hand written summary prepared bymedical officer.
Prepare Final Summaries for the patient with accuracy, add ICDCoding and in-time and print the two copies of the same.
Prepare the certificates i.e MLC Certificate, Rest Certificate,Fitness Certificate, Implant Certificate and fit to fly certificate etc. assuggested by the consultants.
Prepare all procedure reports i.e Cath Lab Procedures andOperative Notes (Minor and Major Operation).
Import the discharge summary in Hinai with medications.
Prepare daily report and submit the same to reporting manager /immediate supervisor.
Work as assigned by reporting manager / immediate supervisor forMT related job in any department.
Positive Communication with all employee, Resident Doctors and Hon.Consultant
Identify mistakes in Summaries / Reports,



and check with doctorsto obtain the correct information.
Provide information to supervisors and co-workers by telephone, inwritten form, e-mail, or in person.
Enter all the history, investigation and prescribe medications inEMR (Electronic Medical Record).
Co-ordinate with Echo Technician for 2D Echo Report of patient andprepare the computerized Report from the Register maintained by Technician andupload the same data in Hinai.
Tomaintain & Document department specific quality indicators.
Any other work assigned by the management from time to time.

Working conditions
Willwork in rotation shifts of 8.5 hours a day.

Skills & Abilities
Verygood computer skills & typing skill.
Ableto communicate with consultants and medical officers.
Basicknowledge of clinical terminologies.
Ableto use hospital information system software.
