Role Overview:
We are looking for a compassionate and qualified Psychologist & Care Manager to provide psychological support, assess patients' mental and emotional well-being, and coordinate their overall care. The role involves working closely with patients, caregivers, healthcare professionals, and internal teams to ensure timely and effective care.
Psychological Care
- Conduct psychological assessments and evaluate patients' mental and emotional well-being.
- Provide counseling and psychological support to patients and their families.
- Develop individualized care and intervention plans based on patient needs.
- Monitor patients' progress and make appropriate recommendations or referrals.
- Maintain accurate and confidential clinical documentation.
- Identify mental health concerns and escalate cases requiring specialized intervention.
- Use evidence-based approaches and maintain professional and ethical standards.
Care Management
- Coordinate patient care across healthcare providers and internal teams.
- Act as a point of contact for patients and caregivers throughout the care journey.
- Follow up with patients regarding appointments, treatment plans, and care requirements.
- Track patient progress, adherence, and outcomes.
- Identify barriers to care and help patients access appropriate healthcare resources.
- Collaborate with physicians, nurses, counselors, and other healthcare professionals.
- Ensure timely communication and appropriate escalation of patient concerns.
Required Qualifications
- Bachelor’s/Master's degree in Psychology, Clinical Psychology, Counseling Psychology, or a related field.
- Relevant certification/licensure as required by applicable regulations.
- Experience in counseling, behavioral health, care coordination, or healthcare is preferred.
- Strong communication and interpersonal skills.
- Robust problem-solving and decision-making abilities.
- Proficiency in MS Office, Google Workspace, and basic healthcare documentation systems.
