Job Summary

As a HCC Medical Coder, you will be responsible for accurately assigning ICD-10 and CPT codes to medical diagnoses and procedures by reviewing outpatient and inpatient medical records. You will ensure compliance with customer and regulatory guidelines while maintaining a coding accuracy of 97% or higher within the defined turnaround time. This role requires robust analytical skills, attention to detail, and a thorough understanding of US healthcare coding standards.

Responsibilities

- Review outpatient and inpatient medical records and assign accurate ICD-10 and CPT codes based on diagnoses and procedures.
- Ensure coding complies with regulatory standards, customer-specific guidelines, and US healthcare compliance requirements.
- Verify medical documentation for completeness and accuracy, identifying discrepancies and following up when clarification is required.
- Maintain a coding accuracy rate of 97% or higher while meeting established turnaround time (TAT) requirements.
- Research and resolve complex or unusual coding scenarios using appropriate coding resources and industry references.
- Participate in coding quality reviews, training sessions, and team meetings to stay updated with the latest coding guidelines and industry changes.

Qualifications and Required Skills

- Bachelor's degree in Life Sciences, Healthcare, Nursing, Pharmacy, or a related field.
- 2-4 years of experience in Medical Coding.
- Strong knowledge of ICD-10, CPT coding guidelines, anatomy, physiology, and medical terminology.
- Experience in HCC (Hierarchical Condition Category) Coding is preferred.
- Good understanding of US healthcare regulations, client-specific processes, and compliance standards.
- Excellent analytical, problem-solving, and attention-to-detail skills.
- Strong verbal and written communication skills.
