About the Opportunity

This role focuses on delivering patient-centred care management, chronic disease support, and seamless care transitions across India. As a Care Manager, you sit at the intersection of clinical care and digital operations - supporting members throughout their healthcare journey by planning, coordinating, and monitoring care. Role Responsibilities

- Assess member health needs and goals to understand clinical risks and care requirements
- Create and manage personalized care plans covering consultations, diagnostics, and preventive care Coordinate appointments and services across doctors, hospitals, and internal teams to ensure smooth care delivery

- Conduct regular follow-ups to track progress, satisfaction, and evolving health needs
- Provide basic health education and guidance to support adherence and better outcomes
- Act as the primary clinical point of contact, resolving member queries and care issues
- Maintain accurate documentation and track outcomes to ensure quality and effectiveness

Skills Qualifications

- Education: BHMS, BAMS, or any healthcare-related degree
- Experience:



1 to 3 years in Patient care, care coordination, or clinical roles preferred

Skills Required:

- Strong clinical understanding
- Good communication and coordination skills
- Organized and detail-oriented
- Comfortable using EMR / healthcare software
