Position Summary
We are seeking a Backend Engineer to build and enhance the Clinical Intelligence & Decisioning platform that powers real-time clinical workflows, decision support, and interoperability across healthcare ecosystems. This role will focus on developing scalable APIs, orchestration services, decision engines, business logic services, event-driven processing, and interoperability capabilities using modern Java technologies.
The ideal candidate will have strong expertise in Java, microservices, healthcare interoperability standards (FHIR, CDS Hooks), clinical workflow integration, and event-driven architectures. The engineer will work closely with product, clinical, interoperability, and platform teams to deliver intelligent healthcare solutions that integrate seamlessly with EHR systems and external healthcare applications.
Key Responsibilities
Clinical Decisioning & Backend Services
Design, develop, and maintain backend services supporting Clinical Intelligence & Decisioning capabilities.
Build scalable microservices that execute clinical business rules, recommendations, alerts, and decision support workflows.
Implement decision services that evaluate patient, provider, and clinical data to generate actionable insights.
Develop reusable healthcare-specific service frameworks and domain components.
CDS Hooks & Clinical Workflow Integration
Design and implement CDS Hooks services for real-time clinical decision support.
Develop hook endpoints, cards, recommendations, suggestions, and clinical guidance services.
Integrate CDS Hooks workflows with EHR systems and clinical applications.
Collaborate with clinical and interoperability teams to ensure recommendations align with care delivery workflows.
Support prefetch optimization and context-aware clinical decisioning.
FHIR & Interoperability
Develop FHIR-based APIs and services to support healthcare interoperability.
Build integrations leveraging HL7 FHIR resources, profiles, and implementation guides.
Implement SMART on FHIR and OAuth-based security models.
Support interoperability between EHR platforms, clinical systems, third-party applications, and enterprise services.
Ensure compliance with healthcare interoperability standards and best practices.
API & Microservices Development
Design and implement secure, scalable REST APIs and microservices using Java and Spring Boot.
Define API contracts and service interfaces aligned to enterprise architecture standards.
Build high-availability services supporting clinical and operational workloads.
Ensure observability, monitoring, performance optimization, and secure API management.
Workflow Orchestration
Develop orchestration services that coordinate complex clinical workflows across multiple healthcare systems.
Implement process automation and service coordination for decision support use cases.
Design resilient service interactions and transaction management patterns across distributed systems.
Enable real-time workflow execution based on clinical events and business rules.
Event Processing & Streaming
Build event-driven architectures to support real-time clinical intelligence.
Develop consumers and producers for healthcare events using messaging and streaming platforms.
Process clinical events and patient data updates to trigger downstream decisioning workflows.
Ensure fault tolerance, scalability, and reliability of event processing services.
Engineering Excellence
Participate in architecture reviews, technical design sessions, and code reviews.
Follow software engineering best practices, including automated testing, CI/CD, and DevOps methodologies.
Contribute to platform modernization, performance optimization, and technical innovation initiatives.
Mentor junior engineers and promote engineering standards across the team.
Required Qualifications
Bachelor's or Master's degree in Computer Science, Engineering, Health Informatics,
or a related field.
5+ years of software engineering experience with Java.
Robust expertise in Java, Spring Boot, and Microservices Architecture.
Hands-on experience building RESTful APIs and distributed backend systems.
Experience implementing healthcare interoperability solutions using FHIR.
Experience developing or integrating CDS Hooks-based solutions.
Strong understanding of clinical workflow integration and healthcare data exchange.
Experience with event-driven architecture and messaging platforms.
Solid understanding of software design patterns and distributed systems.
Experience with SQL and NoSQL databases.
Familiarity with containerization and cloud-native development practices.
Preferred Qualifications
Experience with SMART on FHIR applications and integrations.
Knowledge of HL7 v2, C-CDA, and healthcare interoperability ecosystems.
Experience with clinical decision support platforms and healthcare workflow engines.
Experience working with Epic, Cerner, Meditech, Athenahealth, or other EHR platforms.
Experience with Kafka, Event Hubs, RabbitMQ, or similar event streaming technologies.
Experience with Azure, AWS, or Google Cloud platforms.
Experience with Kubernetes, Docker, and Infrastructure as Code.
Understanding of healthcare regulations, security, and compliance requirements including HIPAA.
